Get Involved — Volunteer!
Interested in volunteering at a school event? Maybe you want to help at a class party. Some of you may even want to attend a field trip as a chaperone. In order to do this you must have the proper documentation and forms.
If you wish to be a chaperone you must have fingerprinting/background check completed AND a 1212 form.
If you wish to be a class parent you must have fingerprinting/background check completed AND a 1212 form.
If you wish to join your student's classroom party, you will need ONLY a 1212 form.
